By submitting this form, I agree to the terms and conditions of EverLoved Emmanuel Center. I understand that I am responsible for any damages to the facility during my event. I agree to comply with all rules, policies, and regulations of the center. I acknowledge that my reservation is subject to approval and availability.

Cancellation Policy:
Cancellation is permitted up to two (2) weeks prior to the event date and may be eligible for a partial refund.

Confirmation Notice:
A confirmation email will be sent including copy of your reservation receipt and signed waiver documents.
